  5. Curriculum Vitae
  6. Post Card Size Whole Body Picture
  7. Birth Certificate (Authenticated by NSO)
  8. College Diploma/Transcript of records (Authenticated by Training Institution/Board
      respectively)
  9. Board Certificate/Board Rating(Authenticated by PRC)
10. Residency Specialty/Diplomate/Fellow Certificate (Authenticated by Training
      Institution/Board respectively)
11. Physical Examination Report
12. Other Requirements:
               a.)  For Male     -  5' 4" in height
                                           Single/married
                                           25-40 years old

               b.)  For Female - 5' 2" in height
                                           Must be single
                                           25-40 years old
