 REQUIREMENTS FOR COMMISSIONSHIP IN THE JAGS, Reserve Force, AFP 1. Application Form duly signed and administered 2. Post Card size picture of the applicant (whole body) and two (2) 2x2 picture 3. Birth Certificate or certification from Local Civil Registrar and/or affidavit of two (2) disinterested persons having knowledge on birth of applicant 4. College Diploma and Transcript of Records or equivalent ROTC/CMT Certificate 5. Bar Certificate 6. Proof of Experience (if professional) 7. Privilege Tax Receipt (if private practitioner) 8. Certificate from employer (if government employee) 9. Bio-data or Curriculum Vitae 10. Affidavit of non-pending case 11. Security Clearance (J2 for Technical Service and Command 2s for Major Services) 12. Physical Examination Report 13. Court Order (if name of applicant has been changed) 14. Certificate of Participation from local government activities such as tree-planting, etc. REQUIREMENTS FOR J2 CLEARANCE: Personal History (PHS) NBI Clearance Police Clearance Mayor's Clearance City or Provincial Fiscal Clearance Barangay Clearance NOTE: All requirements must be submitted in nine (9) copies each, arranged chronologically in folders. Original copies must be presented for the purpose of authentication all photocopies.
