The next phase is
the medical and physical examination, which you will undergo to determine your
fitness for a rigid military training at PMA. In this connection, please report
to the PMA Liaison Office, AFP Medical Center, V. Luna Road,

SCHEDULE OF PHYSICAL/MEDICAL EXAMINATION)
at 1:00 PM.

with you together with the following:

Certificate with two authenticated photocopies
3. Two (2)
photocopies of your Form 137 (High School)

7. ID Pictures: Six
Copies each of 2” x 2” and 1” x 1” (white background)
8. Minimum clothing
for seven (7) days
9. Beddings,
blanket, mosquito net, etc.
10. Two (2) empty
bottles (vial) for laboratory use
11. One (1) pair of
rubber (running) shoes
Enclosed
are the physical exercises that will be administered during the physical fitness
test. Moreover, dental standards require that all dental caries must be filled
and missing teeth replaced with suitable artificial dentures before reporting to
AFP Medical Center.
